Add New Child
To Add the child records,
- Click Add New Child in Child Details Page, Child Info Page 1 will be displayed:
Child Info Page 2
Child Information,
- Enter name in First Name
- Enter name in Middle Name
- Enter name in Last Name
- Select the Date of Birth
- Select Gender
- Enter the School
- Select the Grade from the list.
- Enter the Student ID
- Select the T-Shirt Size from the list.
USDA -CACFP Settings,
- Click the identity in Ethnic Identity
- Select the check box for Racial Identity
Doctor Information,
- Enter Doctor and Hospital details of Name ,Phone Number and Select from the list and Email Address , if required for the child.
Emergency Contacts,
- Enter the Emergency Contacts#1 and Emergency Contacts#2 of Name ,Phone Number and Select from the list, Secondary Phone number and Email Address .
- Enter the notes if any in Notes
Allergy and Special Needs Information,
- Select the Allergy and Special Needs if required for the child.
Child Questions,
- Select Yes or No for the questions which has been asked.
- Click Save to save the record details of child and success message will be displayed.
Note: Enter the data in all the Mandatory fields
