To Add the child records,

Child Information,

  • Enter name in First Name
  • Enter name in Middle Name
  • Enter name in Last Name
  • Select the Date of Birth 
  • Select Gender
  • Enter the School
  • Select the Grade from the list.
  • Enter the Student ID
  • Select the T-Shirt Size from the list.

USDA -CACFP Settings,

  • Click the identity in Ethnic Identity
  • Select the check box for Racial Identity

Doctor Information,

  • Enter Doctor and Hospital details of Name ,Phone Number and Select from the list and Email Address , if required for the child.

Emergency Contacts,

  • Enter the Emergency Contacts#1 and Emergency Contacts#2 of Name ,Phone Number and Select from the list, Secondary Phone number and Email Address .
  • Enter the notes if any in Notes

Allergy and Special Needs Information,

  • Select the Allergy and Special Needs if required for the child.

Child Questions,

  • Select Yes or No for the questions which has been asked.
  • Click Save to save the record details of child and success message will be displayed.

Note: Enter the data in all the Mandatory fields
