To download the family information sheet,

  • Click Download Family Information Sheet in View Forms Page 1. Family Information Sheet Page will be displayed:

Family Information Sheet.

  • Select the required details to be printed.
  • Select the format in which the sheet has to be opened, from the Report Type option.
  • Click Submit the File Download window will be displayed, click Open to open or Save to save the sheet in the selected format.

Note: The sheet will be displayed as per the preferred selection.
